Transitional Care for Faith Community Nurses

The Transitional Care course is designed to provide the faith community nurse with background knowledge on the needs of community members who are in the process of transitioning in care.  This might include from the hospital to home, from the hospital to the nursing home, from the hospital to home with hospice, and many other transitional times.  

15 hours of nursing continuing professional development are being offered by the West Virginia University School of Nursing. The West Virginia University School of Nursing is accredited as a provider of Nursing Continuing Professional Development by the American Nurses Credentialing Center’s Commission on Accreditation.
